Individuals who are NOT affiliated with the University of Chicago (non-UCID holders):
1. You must show one of the following:
- Proof of full vaccination for COVID-19 (14 days after the last dose an FDA or WHO-approved vaccine; approved vaccines are: AstraZeneca, Covishield/Serum Institute of India, Janssen/Johnson & Johnson, Moderna, Pfizer/BioNTech, Sinopharm, and Sinovac)
- Proof of negative COVID-19 test result within 72 hours before the event

2. You must fill out the non-UCID-holder registration form to attend. Please sign up individually.
3. Please be prepared to PRESENT PROOF OF YOUR VACCINATION/COVID TEST (or a picture/digital copy) and A VALID ID at the door. We reserve the right to deny participation to anyone who violates these policies.
**Starting in October of 2022, individuals who have attended a CSDS event occurring on October 1st or later and presented proof of vaccination along with a matching valid ID once will not be required to bring proof of vaccination to subsequent events, so long as you present a matching valid ID. For all individuals who have NOT attended a CSDS event on October 1st or later, you will still be required to present both proof and ID (see section below).
All individuals:
1. Are recommended to wear a mask. (see note below)
2. Must affirm they have read and understood our Code of Conduct and complete a health affirmation form before entering the dance.
To protect you:
1. We will provide wristbands of two colors. One will indicate that attendees feel comfortable being asked to dance by someone whom they do not know. The other will indicate that they are not comfortable with this and that they should not be asked to dance.
2. Time will be allowed between songs to use hand sanitizer.
3. Barring inclement weather, we will keep the windows of the venue open.
Finally, the University of Chicago requires us to include the following statement in any official communication.
This convening is open to all invitees who are compliant with UChicago vaccination requirements and, because of ongoing health risks, particularly to the unvaccinated, participants are expected to adopt the risk mitigation measures (masking and social distancing, etc.) appropriate to their vaccination status as advised by public health officials or to their individual vulnerabilities as advised by a medical professional. Public convening may not be safe for all and carries a risk for contracting COVID-19, particularly for those unvaccinated. Participants will not know the vaccination status of others and should follow appropriate risk mitigation measures.
**NOTE ON MASKING: As of March 4, 2022, UChicago has gone mask-optional in non-academic spaces. We acknowledge that the pandemic is in a period of ebb right now. However, many people remain at risk for severe illness. CSDS seeks to cultivate a community based on mutual care and respect. Consequently, while we no longer require individuals to wear a face mask, we request that all attendees respect the safety of their dance partners and those around them. Please ensure that you are aware of what your partner is comfortable with, especially considering their choice to mask.
