When is outpatient therapy no longer enough?

For therapists, one of the hardest clinical decisions is recognizing when a client may need a higher level of care—without acting too early, waiting too long, or damaging the therapeutic relationship.

A useful starting point is to look beyond diagnosis and assess:

• Current safety and level of risk
• Emotional and behavioral stability
• Daily functioning
• Ability to stabilize between sessions
• Whether the current level of care is still meeting the client’s needs

When the answer becomes unclear, consultation, documentation, and thoughtful level-of-care planning can help clinicians make more ethical and confident decisions.
