WHAT is the program?
The PhilHealth Yaman Ng Kalusugan Program (YAKAP) is a program of PhilHealth, implemented by the Local Government Unit of Hinoba-an in collaboration with the Hinoba-an Primary Care Facility, to help residents access, update, and maximize their PhilHealth benefits.
WHY should you participate?
To ensure that you and your family:
• Are properly registered or updated in PhilHealth
• Can fully enjoy the health benefits you are entitled to
• Have better health coverage and protection
WHERE will the activities take place?
• Day 1: PPAC / Orientation of BHWs & Job Orders
• Day 2: Asia Covered Court
• Day 3: Culipapa Covered Court

WHO can participate?
• Senior Citizens, Persons with Disabilities (PWDs), Indigent Members, Solo Parents and OFW’s who are eligible for PhilHealth benefits in Hinoba-an.
HOW can you participate?
Bring the required documents according to your sector:
👵 For Senior Citizens
• Senior Citizen ID
• Marriage Contract (if married)
• Live Birth Certificate
♿ For Persons with Disabilities (PWDs)
• PWD ID
• Must be encoded in the PWD system
🏠 For Indigent Members
• MDR
• Birth Certificate / Marriage Contract
• Birth Certificate of son/daughter below 20 years old
👩👧 For Solo Parents
• Solo Parent ID
• Birth Certificate of son/daughter below 20 years old
✈️Migrant Workers:
•Latest Contract/OEC
•Valid ID
•Marriage Certificate ( if married)
